The Project EngAGE End of Life Choices Team is proud to present an educational program on the essential end of life documents. Guest speakers will use their legal and medical backgrounds to detail the key differences between the DNR and most forms, including advance directives, such as The Five Wishes document. Please attend to receive your free copy of The Five Wishes booklet. Respite care provided if requested ahead of time.
